Submit Cooperative Eduaction Report

In maintaining the strong academic component of the Case Co-op Program and ensuring quality assignments, all students are required to complete TWO (2) reports while out on assignment. One report is due at the end of each semester.

The reports are reviewed by the Co-op Director and the faculty in your major department. This report should be a minimum of two typed pages.

For confidentiality reasons, your report should be reviewed by your co-op supervisor prior to sending it to campus. Your reports are reviewed by the Co-op Director and the faculty in your major department. A reminder will be mailed to you, which will include information for forwarding your report to your major department.

  • Your current assignments
  • Responsibilities (technical, administrative, other)
  • Special training received
  • Skills and abilities utilized
  • Any integration of coursework to the position
  • Reactions, impressions, and evaluation to date

You may use this form to submit your co-op report or drop off a paper copy in Nord 304. Please submit your report in Microsoft Word or a text-based format.
